BMI is nearly two centuries old: where it came from

Every calculator on this site runs the same short piece of arithmetic that a Belgian statistician wrote down in the early 1830s. The index did not start life as a medical screening tool, and it was not called BMI for close to a century and a half after it was first proposed. Both facts are documented, and both explain a great deal about why the number behaves the way it does today — not as a story to romanticize, but as a plain account of where the formula came from and what it was built to do.

Adolphe Quetelet and the search for the average man

Adolphe Quetelet was a Belgian mathematician, astronomer and statistician working in the first half of the nineteenth century. His central project, later associated with the term social physics, was to apply statistical methods borrowed from astronomy to the study of human populations — looking for regular, predictable patterns in traits such as height and weight when large numbers of people were measured and compared. Around 1832, working with measurement data on Belgian citizens, Quetelet proposed that weight tended to vary in proportion to the square of height across a population, and used the ratio of weight to height squared as a way of describing that relationship. For more than a hundred years afterward, this ratio was known in scientific literature as the Quetelet Index.

Quetelet’s broader ambition, later remembered under the label social physics, was to show that human traits followed the same kind of predictable statistical regularities that astronomers had already found in the physical world. His concept of the “average man” — a composite figure built from measurements taken across a population — became a foundational idea in the early development of statistics, well beyond the single ratio of weight to height. The weight-height relationship was a comparatively small piece of that much larger statistical program, but it is the piece that survived into everyday use two centuries later.

Built to describe populations, not people

The distinction matters more than it might first appear. Quetelet was not trying to assess any one person’s health or body composition — his statistical program was concerned with finding the “average man,” a composite figure representing the central tendency of a population, and with mapping how physical traits distributed around that average across large groups. The weight-height ratio was one tool inside that project, designed to hold up when applied uniformly across a whole population sample, not to capture anything specific about the individual bodies contributing to that sample. Ancel Keys and the name Body Mass Index

The name most people use today comes from Ancel Keys, an American physiologist best known for leading the Seven Countries Study, a large multi-national investigation into diet and cardiovascular health. In 1972, Keys and his co-authors published a paper comparing several candidate weight-height indices — including the Quetelet ratio — against body fat measured independently across large study populations, to see which index tracked body fat most usefully for population-level research. Their conclusion favoured the Quetelet ratio for that purpose, and the paper proposed calling it the Body Mass Index, the name that has stuck ever since. Keys did not invent the arithmetic; he renamed and re-validated a century-old population statistic for a new generation of epidemiological research.

Before Keys: other formulas competed for the same job

The Quetelet ratio was not the only weight-height formula in circulation before 1972. In clinical settings, some practitioners used simpler rules of thumb, such as the Broca index — devised by the French surgeon Paul Broca in the nineteenth century — which set a reference weight in kilograms roughly equal to height in centimetres minus 100. Others used a ponderal index, based on height divided by the cube root of weight rather than weight divided by the square of height. Part of what Keys and his co-authors did in 1972 was compare several of these competing formulas against body fat measured independently across the populations they studied, to see which ratio tracked body fat most usefully at population scale. The Quetelet ratio came out ahead in that comparison, which is the direct reason it was singled out, renamed, and carried forward into the epidemiological research that followed.

What changed between Quetelet’s era and today is not the arithmetic but the scale and purpose of its use. A ratio built for statisticians comparing population data became, over the second half of the twentieth century, a standard figure in clinical practice, public health reporting and, eventually, ordinary conversation about weight. That shift happened gradually, driven largely by the same properties that made the index attractive to Quetelet and Keys in the first place: it needs no special equipment, produces a reproducible number, and can be calculated the same way anywhere in the world. Those properties made it a convenient population-level statistic in the nineteenth century, and they are the same properties that make it a convenient — if limited — individual figure today.
